Job Description

At Crossroads Hospice & Palliative Care, we believe that end-of-life care is about more than medical support—it’s about comfort, dignity, and clarity for patients and their loved ones. Our team stands beside families, guiding them through some of the most meaningful and challenging moments of life.

As a Social Worker, you will learn how to navigate complex conversations around terminal illness, help families understand diagnoses, and connect them with essential resources. You’ll gain experience communicating within an interdisciplinary team that includes nurses, aides, chaplains, and physicians to ensure comprehensive support. With mentorship from seasoned professionals, you'll develop your skills in grief support, legacy work, and emotional care that extends beyond the bedside. This is a place to grow, contribute, and make a lasting impact from the very start of your career.

  • Assist in coordination and participation in Ultimate Gift Programs, such as Gift of a Day and Veteran Recognition.
  • Reliable personal vehicle/transportation, valid driver’s license, and ability to safely operate said vehicle to visit patients and perform job duties within the field.
